My Schedule
Your schedule is managed through Carolina's Connections' approved scheduling and EVV system. Always verify your assigned shifts, client locations, and arrival times through that secure system — not through this website.
Never share client names, addresses, phone numbers, or schedule details through this website or ordinary email.
Clock In / Clock Out
Accurate timekeeping is required for every assignment. You must follow Carolina's Connections' approved timekeeping and EVV procedures to clock in and out for each shift. Unofficial time records are not accepted.
Always clock in and out through the approved system. Contact your supervisor if you experience any timekeeping issues.

Preparation is part of professional care. Before you arrive at your client's home, take a few minutes to review the following.
- Verify your assigned schedule through the approved scheduling system
- Review authorized assignment information and care-plan instructions through the approved secure method
- Confirm your transportation and plan to arrive according to company attendance requirements
- Dress in appropriate, professional attire consistent with the dress code
- Be appropriately prepared for the assignment — bring required supplies or materials
- Follow approved clock-in procedures at the start of your shift
Documentation Center
Document care accurately, completely, and on time. Secure documentation must remain within the company's approved recordkeeping system — do not share completed client records through this website or ordinary email.
Caregiver Visit / Shift Notes
Complete approved shift notes at the end of each visit, accurately recording care provided.
ADL / Assigned Task Documentation
Record ADL and task completion accurately on the appropriate approved forms.
Client Communication
Document relevant client and family communications according to company procedures.
Transportation / Errand Documentation
Record transportation and errand activities, including mileage when applicable.
Medication Reminder Documentation
Document medication reminders provided during your shift as required.
Change-in-Condition Reporting
Report and document any significant change in your client's physical or mental status promptly.
Incident Reporting
Submit required incident reports for falls, injuries, unusual occurrences, or other reportable events according to company procedures.
Never alter, backdate, or falsify documentation. Completed client records must remain within the approved secure recordkeeping system — not on this public website.

For any immediate life-threatening emergency — call 911 FIRST, then follow Carolina's Connections reporting procedures.

Getting Paid Starts With Completing Your Employment & Payroll Documents
The forms below are required as part of your employment with Carolina's Connections. Complete and submit them according to the instructions provided during your onboarding. Contact Administration with any questions.
Payroll Portal
ADMIN: Add your approved payroll portal URL above when ready. Do not collect completed W-4s, I-9s, NC-4s, Social Security numbers, or banking information through this website.

Tells your employer how much federal income tax to withhold from your paycheck. Complete when hired and update whenever your withholding situation changes.

You do not complete a W-2 when you are hired. The W-2 is prepared by your employer — not by you.
Eligible employees receive their W-2 from Carolina's Connections after the close of the tax year, according to applicable IRS requirements. W-2s are typically issued by January 31 for the prior tax year.
When Carolina's Connections' approved payroll portal is connected, eligible employees may access their W-2 through that secure system when available. Contact Administration with any questions about your W-2.

Before You Finish Your Week
Take a few minutes to verify everything is in order before your week closes.
All shifts and timekeeping entries are accurate and submitted through the approved system
Required visit documentation and shift notes are complete and submitted
Transportation and mileage documentation is complete when applicable
Required incident or change-in-condition reports have been submitted appropriately
Upcoming schedule has been reviewed and confirmed
Outstanding training or credential requirements have been addressed
Administration has been contacted regarding any unresolved concerns
